Finding Your Path.

Peer Navigation & Early Intervention Services

Sometimes we want to work with someone who gets it, someone who has been there, and someone who has faced some of the same challenges. Southwest Center’s Peer Navigation Program is for people that are newly diagnosed with HIV or for people that need a little extra help getting back to the doctor to help build the foundation for ongoing HIV treatment and linkage to other helpful services.

The Peer Navigator can help individuals establish medical care, assist in the enrollment for Ryan White or AHCCCS services, attend medical appointments, assist in making behavioral health appointments, and find additional support services to ensure a person doesn’t feel alone in the medical treatment process.

Call 602-307-5330 if you’re newly diagnosed with HIV or have fallen out of care and can use a helping hand establishing medical and other support services!
