Case Management

A Helping Hand.

HIV Medical Case Management

Supportive Case Managers can assist individuals who are living with HIV/AIDS, residing in Maricopa County and utilizing Ryan White services in order to meet medical needs and accomplish healthcare goals when living with HIV. You’ll work together to determine what services are a good fit or that you may be eligible for based upon financial and other eligibility guidelines.

Supportive Case Managers will help you navigate the health care system so that you can receive consistent care and medications; to assist with housing and food needs; and to connect you with additional services such as behavioral health, substance abuse treatment; and other community-based support systems.
