HIV Medical Case Management
Supportive Case Managers can assist individuals who are living with HIV/AIDS, residing in Maricopa County and utilizing Ryan White services in order to meet medical needs and accomplish healthcare goals when living with HIV. You’ll work together to determine what services are a good fit or that you may be eligible for based upon financial and other eligibility guidelines.
Supportive Case Managers will help you navigate the health care system so that you can receive consistent care and medications; to assist with housing and food needs; and to connect you with additional services such as behavioral health, substance abuse treatment; and other community-based support systems. Assistance with completing and submitting eligibility forms is also available.

- Housing Opportunities for Persons with AIDS (HOPWA) assistance for rent and utility costs

- Substance abuse program referrals
– Housing Coordination and Medical Transportation

If you’ve already established care, but find yourself facing challenges maintaining that care and are interested in receiving Southwest Center’s Supportive Case Management Services, the first step is to contact the Ryan White Part A Central Eligibility Office at 602-212-3788 to set up an appointment to see if you qualify. Central Eligibility provides clients with a convenient way to enroll and re-certify their eligibility for all Ryan White Part A services.

Once you’ve enrolled for services through Central Eligibility, you’ll be able to select a location for Case Management Services, including Southwest Center for HIV/AIDS or another agency that you feel would best fit your needs.
